By submitting this e-invoice request, you hereby acknowledge and agree to the following:

1. You are submitting a request for an e-Invoice in accordance with the Income Tax Act 1967 and the relevant Guidelines issued by the Inland Revenue Board of Malaysia (IRBM).
2. All mandatory information provided is complete, true, and accurate to the best of your knowledge.
3. You grant consent to Cuti Cuti Langkawi Sdn Bhd to collect, process, and share the submitted details with the IRBM as required under applicable laws and guidelines.
4. You understand and agree that Cuti Cuti Langkawi Sdn Bhd shall not be held liable for any loss or damages arising from any error, omission, delay, or other matters related to this request or the issuance of the e-Invoice.
5. The e-Invoice will be issued and sent to you within three (3) working days upon receipt of complete and accurate information.
